Through my professional membership with the ATMS, I have a legal requirement to comply with their Code of Conduct, as a Kinesiologist practitioner, please find this attached to my website if desired. When you fill out my intake form through my booking system by Acuity Scheduling, I will ask for your Name, address, mobile, email address the personal information on what it is that you would like to work on with

Policy for Collecting sensitive information from minors:
Sensitive information may be collected from children under the age of 18 under the following circumstances:
in the presence of their parents with their parent or guardian’s full consent
All information collected from minors is securely stored in accordance with this privacy policy.
• Parents, guardians and carers of minors are requested to book the appointment on behalf of the minor under the age of 18.
An adult is required to accompany a minor for the duration of the Kinesiology session
Storage and handling of Sensitive information
I am committed to securely storing and handling your sensitive information.
Sensitive information is stored in a locked filing cabinet and on a password protected computer.
Only I the Kinesiologist practitioner responsible for your treatment will have access to your sensitive material.
I do not store sensitive information online or in the cloud.All archived sensitive information is securely destroyed after 7 years.
Professional Considerations
I am subject to the following regulations regarding the collection of personal information:

I am legally required to identify my clients by collecting their name and address, and failure to provide this information means I cannot offer my services to you.
